What word/phrase annoys you the second you hear it? Why?

"No offense" because its usually followed by something offensive.

Oh really? It sounded positive and funny to me. Never heard it be used patronizing, maybe because we don't speak English here. Oh lol I once read somewhere that with business relations the Dutch didn't understand the English. For instance they said about their idea: oh interesting and the Dutch guy thought: Yay! They find it interesting! But they meant it was a lousy idea LOL.
