Mostly puzzle games come to mind. There's trilogy Gobliiins, Gobliins 2 and Goblin's Quest 3 (written like that because the first game has three main character goblins and second has two).
3 in Three is another puzzle game. Wikipedia explains the story:

Total Distortion is a pile of random shovelware around music video making management strategy. I don't know how to even describe it beyond that.
My real answer to this question is this one:
A honorary mention to PS2 game Ninja Assault. While there's not much really unusual about the game aside being a rail shooter designed for light gun, I feel the basic concept is noteworthy. You are loyal ninjas in medieval Japan out to save princess and the world from demons. For this most important mission you have been given the great secret weapon of your clan. What is the mystical secret power of your ninja clan? Guns.

I've got to disagree with that. Not about weirdest, but about the worst ever. First of all, it's a functioning game, unlike some competitors. Despite simplicity and limited content, it is something you can actually play. Secondly, although it was eventually released separately, it was intended as part of a greater work. So it's more comparable to minigames within actual games and if we count those... it'll be quite easy to find dozens of worse examples.
